Our Consultation form is designed to make sure we schedule your New Guest Appointment correctly, & are prepared when you arrive. Filling out this form coincides with booking an appointment. We will not reach out once this form has been submitted unless your appointment details need to be changed. Once we receive the notification that your New Guest appointment has been requested, we will review your New Guest Form & appointment request.
